Initial Comments:
An off-site follow-up survey conducted on August 23, 2021, found that At Peace Health Care Agency, LLC, had corrected the following deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of an unannounced onsite Medicare recertification and state re-licensure survey, conducted June 23, 2021, June 24, 2021, and off-site on June 25, 2021, June 28, 2021, and June 30, 2021.
